Patient consumed less than 50% of estimated energy requirement for greater than 1 month per family interview and meal observation. Moderate to severe muscle wasting observed at temples, clavicles, and interosseous spaces. Severe subcutaneous fat loss at triceps. No fluid accumulation observed. Handgrip strength not assessed. Impression: findings consistent with severe protein-calorie malnutrition in the context of chronic illness per ASPEN/AND characteristics.

Tolerating supplements. Labs: albumin 2.9, prealbumin low. Nutrition consult note reviewed; agree with dietitian findings of severe protein-calorie malnutrition. Plan: continue nutrition plan, PT/OT evaluation, likely discharge to post-acute care facility.
